  • Parental Consent To affirm that you have read the Pupil Internet Code of Conduct, Pupil Email Code of Conduct, Parent-School Communication Policy and to give or decline consent for your child to be photographed in school.

All Parents need to complete the Parental Consents form

Free School Meal

Contact the school office to complete a Free School Meals form if you think your child may be eligible. No child is automatically given FSM at Junior school so you must apply.

  • Healthcare Plan To provide any medical information regarding your child that the school needs to hold.
  • Administer Medicine First-Aid Trained Staff can administer medicine to your child but only if this form has been completed.

Complete these medical forms if your child needs an inhaler, epi-pen, other medication or if we need to be aware of their healthcare needs,

  • Leave of Absence Request To be completed by any family wishing to apply to the school to take their child out of school during term time.

This form must be completed in advance of any proposed absence during the school day.
