Free School Meals
Contact the school office to complete a Free School Meals form if you think your child may be eligible. No child is automatically given FSM at Junior School so you must apply.

Complete these medical forms if your child needs an inhaler, epi-pen, other medication or if we need to be aware of their healthcare needs,
To provide any medical information regarding your child that the school needs to hold.
First-Aid Trained Staff can administer prescription medicine to your child but only if this form has been completed.
Parent Consent for use of Emergency Salbutamol Inhaler or Epi-pen Auto-injector.

Leave of Absence Request
To be completed by any family wishing to apply to the school to take their child out of school during term time.
This form must be completed in advance of any proposed absence during the school day.
